Fit notes to replace sick notes

A new system of "fit notes" will replace the Med 3 "sick notes" from 6th April 2010.

Sample Fit Note

Unlike the Med 3 form (which has been in use for 60 years), the new form will allow doctors to set out the conditions within which patients would be able to work, and the kind of work they would be able to do.

Doctors do not have to complete this advice, and can simply declare someone unfit for work.  However, doctors can state that the patient "may be fit for work taking account of the following advice..."  doctors may then specify up to four adjustments to work:

  • A phased return to work
  • Altered hours
  • Amended duties
  • Workplace adaptions

Doctors can also record how a patient's condition and treatment may impact upon their work.

Where an employer is able to make the adjustments suggested by a doctor, the patient/employee can return to work.  Where this is not possible, the patient/employee will be considered unfit for work.
